When measured by investigators, the plant’s two Forklifts Nsw each emitted concentrations of CO in excess of 40,000 ppm (recommended guidelines range from 2000 to 10,000 ppm [1-3 ]). On August 17, the plant’s air-conditioning system had been shut down for servicing, and an exhaust fan had malfunctioned, reducing the effective ventilation rate. However, the Forklifts Nsw emitted such excessive amounts of CO that no practical level of ventilation could have maintained CO concentrations below recommended exposure limits.** Neither employees nor managers were aware that the symptoms they experienced were related to CO poisoning, which delayed recognition and response.
